The opportunity

Hi, we're Oscar. We're hiring a Care Coordinator to join our team.

What you'll do

  • Be the primary bridge and entry point for all members and providers working with the Care Management team.

  • Field inbound phone calls from members and providers, using optimized IVR: pathways to ensure accurate routing and striving for one-call resolution.

  • Manage care coordination for members across various programs, including CCM,: TOC, and unassigned populations, across a variety of settings (hospitals, outpatient clinics, home care, supply vendors, dialysis) while maintaining strict confidentiality and the highest level of professionalism.

  • Manage non-emergent medical transportation (NEMT) via UberHealth for members,: strictly following program guardrails to decrease readmissions and improve health outcomes.

  • Handle internal escalations to specialized departments such as Claims, Eligibility, and Care Navigation.

  • Lead special projects as subject matter experts.

  • Collaborate cross-departmentally to create a positive member experience.
  • Perform all other tasks or additional responsibilities.
  • + years of experience with care coordination and navigation in a healthcare setting.
  • Proficient computer literacy skills, with a strong aptitude for using EMRs: (like Medecision) and telephony systems (like InContact) for seamless workflows.
