What you'll do
Manage comprehensive care coordination for senior patients, including: specialist referrals, appointment scheduling, and follow-up with high-risk or disengaged patients
Conduct pre-visit screenings and post-visit check-ins to identify care: priorities, gaps, and ensure continuity across internal and external care teams
Support preventive care by monitoring patient panels, conducting cognitive: screenings, and resolving care gaps through proactive outreach
Provide patient education on chronic disease management using motivational: interviewing and action-planning techniques
Contribute to daily clinic operations, including phone support, front desk: assistance, and maintaining clinical spaces
At least 1 year of experience in a high touch patient-facing healthcare role

- Strong EMR system proficiency and comfort using healthcare technology platforms
- Proficient in Google Workspace (Sheets, Docs, Slides) and adept at navigating: multiple software systems simultaneously
- Excellent written and verbal communication skills with demonstrated ability: to collaborate effectively on diverse, multi-disciplinary teams
- Self-motivated with proven ability to work independently, adapt to change,: and proactively address challenges under pressure
