What you'll do
Conducting virtual visits across a broad spectrum of primary care needs: including acute, chronic, preventive, and mental health care
Establishing strong patient relationships to drive continuity of care
Collaborating with virtual care teams, health coaches, behavioral health: specialists, and consulting physicians to coordinate comprehensive care
Participating in weekly Clinical Rounds and continuous learning through other modalities
Supervising one or more NP or PA colleagues as part of a virtual care team
Contributing to the development and refinement of virtual panel-building workflows and best practices
What they're looking for
- Build and maintain a sustainable patient panel, achieving schedule utilization benchmarks
- Enrolled in, or have completed, an accredited Internal or Family Medicine residency program
- Practiced at least 2 of the last 5 years in an outpatient primary care setting
- Board Certified in Internal or Family Medicine, or Board Eligible with plans: to obtain board certification within 1 year of your One Medical start date
